Annual Open Enrollment 2009 from October 20 to November 14, 2008. Employees will receive enrollment materials in Campus Mail the week of October 13, 2008.

Choosing a plan and helpful information
Open Enrollment is the employee's annual opportunity to make changes in plans listed below for an effective date of January 1, 2009:
- Medical plan
- Dental plan
- Personal Accident Insurance
- Tax Saver Benefit (TSB) Plan
- Wellness HRA Plan
In mid-October 2008 employees will receive:
- An Open Enrollment booklet informing them of details such as plan changes and rates, and
- A statement informing them of the plans in which they are currently enrolled.
October 20 - November 3, 2008: Open Enrollment information sessions will be held at all campus locations listed below.
November 14, 2008: This is the deadline for Open Enrollment 2009.

MEDICAL PLANS
IU PPO-Plus
- As announced last year, this plan will be discontinued effective December 31, 2008. Current enrollees will be defaulted to the Blue Preferred Primary POS plan effective January 1, 2009, unless the employee selects a different plan.
IU PPO $900 Deductible
- The Organ and Tissue Transplant maximum will increase to $2 million.
- Prescription copays will increase to $8 generic/$20 low-cost brand/$40 high-cost brand for retail and $20/$45/$90 for mail order.
- Medical annual out-of-pocket copay maximums will increase to $1,500 per individual and $4,000 per family for in-network services with a separate out-of-pocket maximum for out-of-network services. In order to facilitate comparison of plans, the out-of-pocket maximums will be restated to include copays and the deductible, that is: $2,400 ($900 deductible plus $1,500 in copays) for individual coverage, and $6,700 ($2,700 deductible plus $4,000 in copays) for the family out-of-pocket maximum.
Blue Preferred Primary POS
- The Organ and Tissue Transplant maximum will increase to $2 million.
- The lifetime maximum of $5 million per member for medical coverage will be eliminated.
- Prescription copays will increase to $20 generic/$45 formulary brand/$90 non-formulary brand for mail order.
- Medical copays will increase to $25 for office visits and $100 for outpatient services.
- Medical annual out-of-pocket maximums will increase to $5,000 per family for in-network services and to $10,000 per family for out-of-network services.
IU HDHP PPO & Medical Savings Plan
- This is a new plan option effective January 1, 2009, that includes both medical coverage and a unique tax-advantaged savings account for medical expenses, as provided by IRS regulations. In that the provisions are very different than current plans, an overview of the plan is provided on pages 3-4 of the Open Enrollment booklet.
Health Risk Assessment and 24-hour Nurse Line
- These new features are being added for all IU-sponsored medical plans. Included are an online Health Risk Assessment and phone access to a registered nurse for health-related questions 24 hours a day, 7 days a week. These features will be provided through Clarian Health.
IU DENTAL PLAN
- Coverage will be enhanced by adding coverage for dental implants.
PERSONAL ACCIDENT INSURANCE (PAI)
- Premiums will decrease 20 percent for employee coverage and 15 percent for family coverage.
Tax Saver Benefit (TSB) and Wellness HRA Plans
- For those employees who enroll in the IU HDHP PPO & Medical Savings Plan and the TSB and/or Wellness HRA Plans, IRS regulations limit the types of claims that can be paid from these account. Eligible reimbursements are limited to:
- Dental and vision services; and
- Medical services received after the HDHP deductible is met.
- IU HDHP PPO & Medical Savings Plan participants will need to adjust their annual TSB healthcare reimbursement account pledges accordingly.
