2008 Medical Care Plans IU PPO-Plus
No New Enrollments for 2008
At its inception in 2001, the IU PPO-Plus plan provided a reasonable option to employees who wished to budget their medical costs through payroll premium contributions rather than through a plan deductible. Although this no-deductible plan initially offered a valuable choice for employees, in recent years its reasonableness has been eroded by disproportionately large premium increases.
For this reason, no new enrollments will be accepted into this plan for 2008, and barring a significant decrease in the employee premiums for 2009, the plan will be discontinued effective December 31, 2008. Although current participants can remain enrolled for 2008, they are encouraged to review this plan’s costs compared to the IU PPO $900 Deductible and Blue Preferred Primary POS plans which are more cost-effective.
Medical Services
This plan is available to all eligible Indiana University employees, regardless of area of residence. When network providers are used, participants receive full benefits. Partial benefits are received from non-network providers, except in the case of an emergency. Network providers include Anthem Blue Access PPO providers in Indiana, Ohio, and Kentucky and BCBS Blue Card providers in other states.

Obtain information about precertification of a medical procedure by calling Anthem Utilization Review at 800-345-4348.
Prescription drugs are obtained from a participating pharmacy. Participants are responsible for copays at the time of service. Participating pharmacies include most retail pharmacy chains and some independent stores. Mail order prescriptions are also available from Anthem Rx Direct.

Mental Health and Chemical Dependency
IU PPO-Plus offers full benefits from Indiana University Psychiatric Management (IUPM) and partial benefits from out-of-network providers. All services must have prior authorization by IUPM in order to be covered.

Select a mental health professional from the IUPM Provider Network (click the Search for a Provider! link).

Print the ID card: Mental Health and Chemical Dependency Coverage.

In case of an emergency: Contact IUPM or obtain care from the nearest qualified mental health professional, who must then call IUPM within 24 hours of care.
